The Health and Welfare Fund provides disability, medical, vision, dental  benefits, retiree, death and Accidental Death and Dismemberment (AD&D) benefits on a self-funded basis.

The Summary Plan Description describes the benefits to which you are entitled, the eligibility rules, and the procedures you should follow in order to obtain benefits through the Plan. We urge you to become familiar with the benefit program and to bookmark this Web Site for future reference.

The more detailed provision of your medical benefits available and terms and conditions of such may be found in the Summary Plan Description.

Occasionally changes are made to the Summary Plan Description. The administrator will send letters documenting the changes. Please keep these changes with your own SPD.
