Your Prescription Drug Benefit Program under The Brewery and Beverage Teamsters 67  Health & Welfare Fund prescription drug program is administered by Caremark Inc., administrator of the retail pharmacy program, and provider of the mail order service pharmacy program.

Prescription Drug Program Overview

Features	Retail Program	Mail Service Pharmacy
When to Use	When you need a prescription on a short term basis, (for example, an antibiotic to treat a sore throat)	For prescriptions you use on a regular basis (for example, medication to reduce blood pressure)
Supply Per Prescription/Refill	Up to 30 days	Up to 90 days
Maximum Allowable	As of June 1 2008 the annual Cap was raised to $20, 000 per family member
Your Cost Per Prescription/Refill	Participating pharmacies $7.00 co-payment per generic medication up to a 30 day supply $20.00 co-payment per brand name medication (listed on the Caremark Primary Drug List) up to a 30 day supply $35.00 co-payment per brand name medication (not listed on the Caremark Primary Drug List) up to a 30 day supply $100.00 co-payment on Specialty drugs. Drugs must be approved through Caremark (SGM) program and purchased through Caremark pharmacy.	If this is your first prescription, ask your doctor for two prescriptions: the first for a 14 day supply to be filled at a participating retail pharmacy; the second for the balance, up to a 90 day supply, to be filled through the mail service pharmacy $14.00 co-payment per generic medication up to a 90 day supply $40.00 co-payment per brand name medication (listed on the Caremark Primary Drug List) up to a 90 day supply $70.00 co-payment per brand name medication (not listed on the Caremark Primary Drug List) up to a 30 day supply

Caremark Customer Care Representatives are available 24 hours a day 7 day a week.
